Expiring & Replacing Existing Consent Forms
Requesting a New Consent Form
1. Navigate to the Global Nav and open the Appointment.
2. Go to the GFE tab within an existing appointment. Scroll past the summary, and GFE notes until you see the Form section.
3. Click the three dots (...) on the top-right corner of the Forms area > select “Require New Consent Forms.”
4. A pop-up will list available consent forms; choose the forms needed > Yes, request.
5. Once requested, you or your patient can open, review, and sign the form immediately.
This is great for last-minute approvals, such as a retail product requiring a consent form.
Expiring & Replacing Existing Consent Forms
If an existing content form needs to be replaced for any reason, you can expire and replace the form by following the steps below.
1. Go to the GFE tab within an appointment or the patient profile page. Scroll past the summary, and GFE notes until you see the Form section.
💡 Tip - Whether you navigate to the patient profile or an appointment details page, the process of managing consent forms remains the same.
2. From the Forms section, click the three dots and select Require new consent forms.
3. On the popup under the Expire existing consent forms... section, you'll see all signed and unsigned consent forms currently attached to the patient. select the form to expire > Yes, request to issue a new consent form.
✏️ Note - In this step, you can also request additional new consent forms by selecting them from the Create new consent forms... section.
4. The form will expire, and the replacement form will be added to the patient's forms list to sign.
Archiving Forms
1. Go to the GFE tab within an appointment or the patient profile page. Scroll past the summary, and GFE notes until you see the Form section.
2 From the Forms section, click the three dots and select Manually archive consent forms
3. Pick which forms to archive > Yes, Archive. These forms disappear from the patient's active forms list.
