TDDD License - Ohio Providers

What is a TDDD license?

This is a license that Ohio providers should obtain per state law. A Terminal Distributor of Dangerous Drugs (TDDD) is a person (individual, partnership, association, limited liability company, corporation or government agency) who is engaged in the sale of dangerous drugs at retail, or any person, other than a wholesale distributor or a pharmacist, who has possession, custody, or control of dangerous drugs for any purpose other than for that person’s own use and consumption.

Do I need a TDDD license?

RN: If you are an RN you will absolutely need this license.

NP or PA: If you are an NP or PA, we strongly encourage you to obtain this license. You may fall under an exemption but you'll need to check with the state to confirm this.

What Category is my business considered?

If you are offering Tox, Filler, and/or IV, this should be Category 2. You may fall under Category 3 if you have weight loss drugs or others that are similar. Please check with the state's guidelines to confirm your category.

How much does a TDDD license cost?

As a provider with your own professional association, corporation, partnership or LLC, the TDDD license cost ~$120. 

How long does it take to obtain a TDDD license?

This process can take up to 60 days to complete. During the first two weeks, your application will be under review. After this, you as the provider will be contacted to schedule an inspection of your MedSpa premise. Click here to access the inspection guide.

Is my Medical Director involved in this process?

Your Medical Director will need to be listed on your application. While you sign the application, your MD is listed on the application. Please check with the state's office on who should be listed as the purchaser vs prescriber.
