How to manually expire an intake form
Overview
In Moxie Suite, intake forms can expire, ensuring that client information stays current and accurate. When an intake form expires, any associated Good Faith Exams (GFEs) assessments are voided, and a new GFE must be completed. Intake forms can expire in one of three ways:
Automatic Expiry:
365 days after the client initially fills out the form.
New Submission:
When the client submits a new version of the intake form through their client portal.
Manual Expiry:
When a provider manually selects the option to "require new intake forms." In this case, the client is notified via email to resubmit their forms. Let's review how you can manually expire an intake form in the next section.
How to manually expire an intake form
1. Log in to your Moxie Suite account using this link.
2. Click Clients in the Global Nav.
3. Search for the client by name or phone number in the search bar then click on their client card.
4. On the Client details page click on the GFE tab.
5. On this page click the ellipses to the right of the GFE section > Require new intake forms.
6. A pop-up will appear asking you to confirm the expiration request. Click Yes, request to proceed with the expiration, or Cancel to go back.
✏️ Note - When manually requiring new intake forms, the client will receive an email notification. This email will guide them to log into their client portal and complete the new forms.
❗Important - Once the client submits their new intake forms, submit/conduct a new GFE to keep their records up-to-date.
