Moxie fee structure update

• What's changed:
  • Provider owner tips will no longer be included in the 9% fee.

Estimated appointment value

• What changed:
  • On the appointment, you can now see an estimated appointment value. This will add up all service+product prices so you have a total estimated invoice value before you go to checkout. It also gives you a preview of the relevant wallet credits. I.e. all points/dollar credits will be shown, and any service/product credits that are relevant to the appointment today.
